When is ICSI recommended?
ICSI may be recommended in case of severe male infertility issues, such as low sperm count or low sperm motility.
If the male partner has low sperm count, ICSI may be recommended to increase chances of fertilization.
If the male partner has decreased sperm motility, or abnormalities in sperm morphology (shape), directly injecting a single sperm into each egg, can bypass natural barriers to fertilization
In cases where sperm cannot be obtained through ejaculation due to conditions like obstructive azoospermia or non-obstructive azoospermia, surgical sperm retrieval techniques such as testicular sperm extraction (TESE) or microdissection testicular sperm extraction (micro-TESE) may be used in conjunction with ICSI to retrieve viable sperm directly from the testes.
In some cases of unexplained infertility where conventional IVF has not resulted in successful conception, ICSI may be recommended as a way to ensure successful fertilization by directly injecting sperm into the eggs.
Same sex women couples or a woman without a male partner may choose donor sperm to conceive using IUI treatment. The donor sperm used is from ART certified and registered sperm which has been tested for quality.
If previous attempts at conventional IVF have resulted in failed fertilization despite the presence of apparently normal sperm and eggs, ICSI may be recommended as an alternative approach to increase the chances of successful fertilization.
In cases where the female partner is of advanced maternal age or has poor egg quality, ICSI may be used to maximize the chances of successful fertilization by directly injecting sperm into the eggs, thus overcoming any potential barriers to fertilization.

Infertility Assessment
Before the start of treatment, severity and type of infertility is understood. This has three steps-

EGG RETRIEVAL
Just like a normal IVF procedure, Egg retrieval in ICSI procedure is also done. After ovarian stimulation through medication, and monitoring the ovulation, a trigger injection of human chorionic gonadotropin (hCG) or a GnRH agonist is administered to induce final egg maturation. Approximately 36 hours later, the eggs are retrieved from the ovaries using transvaginal ultrasound-guided aspiration.

SPERM EXTRACTION
Before the ICSI procedure begins, a semen sample is collected from the male partner. If necessary, the sperm sample may undergo processing in the laboratory to isolate motile and morphologically normal sperm from seminal fluid and debris. In cases of difficulty in sperm retrieval, sperm is retrieved through a treatment named TESA.

SPERM INJECTION INTO EGG
The selected sperm is carefully injected directly into the cytoplasm (inner part) of the egg using the micropipette. The embryologist ensures precise positioning of the sperm within the egg to maximize the chances of successful fertilization.

MONITORING EMBRYO
Fertilized eggs (embryos) are cultured in the laboratory for 3 to 5 days, as they develop into multicellular embryos. During this time, embryologists monitor embryo development and quality to select the most viable embryos for transfer.

EMBRYO TRANSFER
One or more selected embryos are transferred into the woman's uterus. The number of embryos transferred depends on factors like the woman's age, embryo quality, and previous IVF treatment history.
ICSI Procedure
After the assessment is done and patient is declared medically fit to undergo the treatment, ICSI procedure will begin
Test for Pregnancy
Approximately 10-14 days after embryo transfer, a pregnancy test is performed to determine whether implantation has occurred and pregnancy has been achieved. If the test is positive, ongoing prenatal care is initiated to monitor the progress of the pregnancy.
